Supporting Advanced Primary Care Management workflows.
Since January 2025, Medicare pays for Advanced Primary Care Management (APCM) through three monthly codes. The services are not time-based, but each month requires a set of care management capabilities. Weekly check-ins and a physician dashboard support several of them. Eligibility and billing decisions stay with your practice.
The three APCM codes
| Code | Patients | Approximate 2026 national payment |
|---|---|---|
| G0556 | One or no chronic condition | About $16 per month |
| G0557 | Two or more chronic conditions at significant risk | About $54 per month |
| G0558 | Qualified Medicare Beneficiaries meeting the G0557 criteria | About $117 per month |
National averages for illustration. Actual payment varies by locality and requires meeting all CMS requirements.
Where weekly check-ins help
- Enhanced communication: asynchronous check-ins and messages beyond phone calls
- Care plan follow-up: goals set by your physicians, tracked every week
- Population-level management: one view of who is on track and who needs attention
- Documentation: a dated record of follow-up activity for each patient
What stays with your practice
- Patient consent, documented in the medical record
- The initiating visit for new patients
- 24/7 access for urgent needs and care transitions
- Billing, eligibility and the clinical decisions behind each service
A link with value-based care
APCM includes a performance measurement requirement that practices can meet through the Value in Primary Care MIPS Value Pathway or by participating in an eligible ACO, such as an MSSP or ACO REACH organization.
Questions
APCM is not time-based. Services are furnished by the billing practitioner and clinical staff under their direction. Automated check-ins support that work; they do not replace it.
No. Payment depends on eligibility, consent, documentation and the requirements CMS sets for each month. We help with the follow-up and the record, not with billing decisions.
