Billing support for the follow-up your team already does.
In many health systems, follow-up between visits can be reimbursed when it is documented. Feroce keeps a dated record of every check-in, reminder and care plan step, so your team has the documentation these programs ask for. Eligibility and billing decisions stay with your practice.
What Feroce documents for each patient
- Every check-in, with the date and the patient’s answers
- Care plan goals set by your physicians, and their progress
- Reminders sent for refills, labs and next visits

What stays with your practice
- Patient consent, documented in the medical record
- The initiating visit for new patients
- 24/7 access for urgent needs and care transitions
- Billing, eligibility and the clinical decisions behind each service
A link with value-based care
APCM includes a performance measurement requirement that practices can meet through the Value in Primary Care MIPS Value Pathway or by participating in an eligible ACO, such as an MSSP or ACO REACH organization.
United States: Chronic Care Management (CCM)
CCM is billed on clinical staff time, and automated messages do not count as staff time. The Feroce record supports the documentation of the time your team spends with each patient.
Outside the US
Reimbursement for remote follow-up depends on each country’s rules. We review the options that apply to your organization with you before launch.
Questions
APCM is not time-based. Services are furnished by the billing practitioner and clinical staff under their direction. Automated check-ins support that work; they do not replace it.
No. Payment depends on eligibility, consent, documentation and the rules of each program. We help with the follow-up and the record, not with billing decisions.
See your patients’ weeks, not just their visits.
